Practicefirst would like to remind you that Emblem Health will be changing their ID cards and member IDs throughout 2018.
The new ID will start with the letter “K” followed by a unique eight digit number.
The final two digits of the new ID will refer to the subscriber and dependents. For example, the subscriber will be “00” and the first dependent will be “01.”
Example of the new format: K1234567800
For claims, use the ID number that was active on the date of service. Please note that Medicaid/HARP numbers are not changing at this time.
