Independent Health is in the process of issuing new provider agreements for all primary care providers that will become effective January 1st, 2018.
These new participating provider agreements for primary care physicians have been distributed via US Mail throughout August, pending final approval from the New York State Department of Health.
The purpose of the new agreements is to meet all existing regulatory requirements, address the transition to Primary Value, capture all previous contract amendments and ensure consistent language among participating providers.
Upon receiving the new participating provider agreement, please review it carefully, sign it where indicated and return the entire document to Independent Health as soon as possible. Please note that the entire signed agreement must be returned to Independent Health. They will not accept just signature pages.
Independent Health will also be sending out new agreements for all nurse practitioners and physician assistants in early September.
