*Most health insurance companies use Council for Affordable Quality Healthcare (CAQH) when credentialing and re-credentialing individual practitioners. It is important that you keep your information on CAQH current. Update your CAQH profile regularly with license(s), malpractice, demographic information, disclosure questions and attest quarterly to the information within the profile.
